*POSITION SUMMARY: *Siskiyou Community Health Center’s (SCHC’s) Outreach Program envisions healthy, resilient families where children flourish and arrive to school ready to learn.
The Outreach Program strengthens families in the local community from pregnancy to early childhood through education that promotes healthy, thriving children and nurturing relationships.
The programs deliver services primarily through a certified home visiting model using evidence-based curriculum.
The Outreach Facility Manager is responsible for the day-today, hands-on management of the Outreach program site(s).
The individual in this role is involved with overseeing employees at the site, providing clinical and reflective supervision, and managing facility needs.
The Site Manager also supports the implementation of site specific grants including Healthy Families, Maternal, Infant, and Early Childhood Home Visiting (MIECHV), Project Baby Check, and Healthy Start.
The Site Manager assists the Director of Outreach in activities including staff training, development and program evaluation.
They support strong, ongoing relationships with community and state partners.
The Outreach Site Manager will work closely with the Director of Outreach and site Supervisor(s) to ensure the overall quality of program services including monitoring and balancing caseloads, overseeing documentation and claims, and assuring compliance with safety procedures and organizational policies regarding the provision of Outreach services.
*DUTIES and RESPONSIBILITIES:* • Support oversight of the site’s specific Outreach programs, as applicable for the site’s service area, potentially including Healthy Families, MIECHV, Healthy Start, and Project Baby Check.
• Oversee operations of the physical site, including any facility needs, repairs, or concerns.
• Lead and direct employees at the site in any location-based facility emergency situation.
• Provide clinical and reflective supervision to site employees, in coordination with Outreach Program supervision caseload requirements, including mentoring and staff development.
• Supports site employees to develop skills and reach individual performance goals and objectives.
• Supports individualized orientation plans for new employees, ensuring proper and comprehensive training.
• Assumes responsibility for safety of families, visitors, and employees at designated sites.
Maintains current knowledge of disaster procedures, Occupational Safety and Health Administration (OSHA) requirements, and emergency procedures.
• Works in coordination with the Director of Outreach to respond to client emergencies including crisis intervention, mandatory abuse reporting, and/or domestic violence situations.
• Supports resource allocation and community connections/referrals as appropriate.
• Audits charts for employees at the site location.
• Reviews and balances caseloads.
• Oversees clinical records and documentation, ensuring best practices, professional ethics, and program requirements are adhered to.
• Collaborates with the Director of Outreach and/or site supervisors (as applicable) to resolve any identified Human Resources concerns or needs for employees at the site location.
• Oversees timecards, vacation requests, and schedules of employees stationed at the facility location, ensuring adequate facility coverage at all times.
• Collaborates with the Director of Outreach to complete annual performance evaluations for site employees.
• Promptly notifies the Director of Outreach when any concerns are identified for site employees, including performance concerns, poor caseload engagement or retention, interpersonal concerns, or other human resources issues.
• Supports the implementation of programmatic requirements and/or changes identified by grant funded programs in collaboration with the Director of Outreach.
• Implements the policies and procedures in the Outreach program manual.
• Supports the Director of Outreach in activities required to monitor grant budgets and Medicaid Administrative Claiming, as well as leverage any additional revenue resources.
• Assists with fundraising to meet in-kind and match goals.
• Works with all SCHC Outreach locations to support systematic efforts are made to meet Outreach Program performance standards and indicators.
• Supports the Director of Outreach to complete site assessment and reaccreditation activities.
• Attends monthly webinars and/or training programs as required for grant funded programs.
• Supports and monitor the program training plan for site employees.
• Works with local Advisory Committees in coordination with the Director of Outreach to support connections with the local Early Learning Hub, Coordinated Care Organizations, Early Childhood team, and other appropriate community agencies and partners.
• Supports data collection for Quality Improvement activities, in accordance with the quality assurance calendar.
• Collect, analyze, and report data for the site regarding program statistics and demographics to the Director of Outreach.
• Conducts Quality Assurance shadowing of home visitors at the site location.
• Shares concerns related to program quality with the Director of Outreach and support the implementation of quality improvement action plans.
• Coordinates with other local agencies providing services to young children in order to share resources, avoid duplication of services, and assess community needs.
Promotes principles of equity, diversity, and inclusion throughout the program.
Leads with humility.
• Meets regularly with the Director of Outreach, site supervisors (if applicable), and home visitors at each site they oversee to ensure effective flow of information.
• Promotes positive inter-departmental relationships.
• Promotes preventative health and wellness through the use of integrated services available to SCHC as a Federally Qualified Health Center (FQHC).
• All other duties as assigned.
*QUALIFICATIONS, EDUCATION, and EXPERIENCE:* • High School diploma or equivalent minimum.
• Bachelor’s degree with related work with children and families or 3 years of relevant experience, or less than a bachelor’s degree but with commensurate Healthy Families America or Outreach Program experience.
• Minimum two (2) years of full-time experience working with children and families.
• Solid understanding and experience in managing a diverse staff with humility.
• Willingness to engage in building reflective practice (e.
g.
, capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision.
• Experience working within Healthy Families America (preferably Health Families Oregon) desired.
• Proof of current CPR certification or ability to obtain one within 90 days.
• Proof of current unrestricted Oregon driver’s license, comprehensive automobile insurance, and a safe driving record.
• Use of private automobile in good, working condition to drive to outlying rural areas in possibly hazardous weather and/or road conditions.
• Proof of current immunizations that meet clinic health and immunization standards.
• Required to submit to pre-employment criminal background investigation.
