*Job Description:*
As an Infusion Nurse II at Option Care Health, you'll play a crucial role in coordinating and delivering intermediate direct patient care to individuals in their homes or alternate infusion suites.
Your responsibilities include:
* _Innovative Patient Care:_ Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress towards goals, perform treatments, administer medications and ensure patient safety and continuity.
* _Empower through Education:_ Educate patients and families, providing hands-on and verbal instruction for prescribed therapy.
Assess patient needs through each visit, re-evaluate needs, responses to therapies and supplemental physician orders.
* _Effective Communication:_ Collaborate with the clinical team, medical providers, and patients to facilitate seamless continuity of care.
This will include coordinating discharge planning.
*Qualifications:*
* Active RN License: Unrestricted license in the state of practice.
* Experience: Minimum of 1 year of nursing experience.
* CPR Certification: Current certification required.
*Other Requirements: *
* Willing to travel up to % of the time for business purposes.
* Participates in nursing department on-call responsibilities.
