Telephone Triage RN

Telephone Triage RN Full time preferred (8 hour shift preferred 10 hour shift acceptable) Location: Not remote.
La Clinica Center for Learning and Innovation Responsibilities: Providing telephone and electronic communication with patients and the clinical team.
Provide clinical decision support to population health team.
Support primary health center RN’s in with Transitional Care Management for patients.
Able to work with patients of various socio-economic backgrounds and temperaments; demonstrates tact, diplomacy and compassion in interactions with patients and staff.
Preferred: Spanish speaking preferred but not required.
Telephone Triage certification preferred.
Required: Nursing experience: 2 years Degree: Bachelor’s or associate degree in nursing.
