*Hearthstone Nursing & Rehabilitation Center is seeking a motivated and experienced Wound Care Nurse to join our team!* In this position, you will act as the coordinator of the center’s wound team to oversee the general application of the skin management program, complete resident assessments, create and implement care plans, etc.
*Responsibilities* * Must adhere to Code of Conduct and Business Ethics policy, including documentation and reporting responsibilities.
* Participate in Quality Improvement activities as assigned.
* Ability to understand and apply training from in-service education and ability to instruct personnel during training education and staff meetings.
* This position regularly requires long hours and frequent weekend work.
* This is a position in a health care facility open 24 hours a day, 365 days a year.
Rotating shifts as well as holiday and working during inclement weather may be required.
Work Environment: This job operates in a fast-paced nursing home environment.
Also routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The responsibilities of this position involve significant physical activities including standing, lifting (up to 50 pounds unassisted), bending, stooping, pushing, pulling and twisting.
All employees of skilled nursing facilities may be required to provide lifting and transfer assistance to residents.
Lifting and/or transferring some residents will require use of a lifting device and /or assistance of other staff.
While performing the duties of this job, the employee is regularly required to talk or hear.
The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
*Required Education and Experience:* * Must hold and maintain a current license to practice as a Registered Nurse in practicing state.
* Must have experience in Wound Care.
* Recent clinical experience, education, or specialty skills specific to geriatrics.
* Demonstrate working knowledge of infection control principles and practices.
* Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth anddevelopment.
* Proven ability to communicate effectively and positively with staff, residents and family members.
* Ability to adapt to a flexible work schedule.
* Ability to relate positively, effectively and appropriately with residents, families, community members, volunteers and other facility staff.
* Possess special interest in, and a positive attitude about, working with long-term care residents and the elderly.
* Ability to read, write, speak and understand English.
* Meet all health requirements, as required by law.
* Must be able capable of maintaining regular attendance.
* Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
* Demonstrate understanding of State and Federal rules and regulations that govern the practices within the facility.
*Preferred Education and Experience* * Experience in a fast-paced environment.
* Post-acute care or health care experience preferred.
* Some wound care experience is needed.
