Summary
Working in collaboration with the clinic care teams, the Quality Improvement Manager will partner with patients, providers, and payers to implement standard work processed that drive improvement in quality and patient outcomes.
This role will apply value-based care standards and utilize standard analytics tools to drive performance.
The goal of the Quality Improvement Manager is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.
Essential Duties
Coordinates with ACOs, PCPCH, & Health Plans on meeting APM goals
Review payer gap lists to identify patients that need care gaps closed.
Act as a consultant to medical practices on PCPCH
Learns and understands Primary Care practice workflows with respect to optimal and coordinated health care for target patient populations.
Support practice staff to develop creative processes to proactively manage target population and provide data support and population health education when applicable.
Perform chart reviews at all assigned sites/practices to identify and close gaps in care as needed.
Including communication of findings to providers or payers
Education & Experience
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Three years of clinical healthcare experience or three years of experience working with HEDIS quality measure.
Self-directed and able to perform independently.
Physical Demands & Work Environment
The employee must occasionally lift and/or move up to 10 pounds.
While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel and talk or hear.
The employee is occasionally required to stand; walk and reach with hands and arms.
